Tile installation Description

I am about to install tile in my entry way. I have removed carpet and (http://maplered.com) vinyl tile which was on top of a concrete slab. I have a couple of questions. First, after removal of the Tiles (http://www.g682.net) , I was not able to get all the adhesive off the concrete. I bought a latex primer to increase the bonding of the concrete self leveling fill product I bought (my floor is not level). Do I need to do anything else to prepare the floor (It is "clean"). Second, the floor is below grade, and (http://maplered.com) gets damp by the exterior wall, (ie. the baseboard wood rotted). Is there a particular membrane I should put down? I guess that I should put primer down, then concrete fill, then an adhesive for the membrane (any special adhesive?), then the membrane, and (http://maplered.com) finally thinset for the Tiles (http://www.g682.net) . Is this correct?
